Please share information about physical activity policies and practices at your school.
The link below will take you to the online survey, which will take about 20 minutes to
complete. After completing the survey, you can add your email to win one of 15 $25 gift
cards.
At this time, we are only surveying teachers in Arizona elementary schools (or schools
that serve children in grades K-6 including K-8 and K-12). Stay tuned for another survey
opportunity for secondary teachers this spring.
